Navigating the NDIS is tricky, and many people don’t know where to start with their plan. That’s where we come in - the best way to describe our role as Support Coordinators, is kind of like being your NDIS personal assistant. Our services do not cost you anything out of your own pocket, they are paid for by the NDIS out of your plan.

NDIS Providers

We are registered NDIS providers who:

  • Work closely with participants, and their informal, formal and mainstream supports to provide independent support, and coordinate all aspects of their NDIS plan.
  • Pride ourselves on being able to effectively research and establish links with a range of community, mainstream and funded support services as needed.
  • Provide quality, capacity building support, assisting participants to:
  • source and access support options and connect with services
  • coordinate services
  • implement, monitor and manage plans
  • build skills and confidence
  • respond to and deal with crisis'
  • work towards their goals
  • Report outcomes to the NDIS and assist with pre-planning prior to plan review

CONNECTION & COORDINATION

There are three levels of support coordination that we can assist you with:

  • Support connection – This support is to build your ability to connect with informal, community and funded supports enabling you to get the most out of your plan and achieve your goals.
  • Support coordination (Level 2)– This support will assist you to build the skills you need to understand, implement and use your plan. We will work with you to ensure a mix of supports are used to increase your capacity to maintain relationships, manage service delivery tasks, live more independently and be included in your community.
  • Specialist Support Coordination - This is a higher level of support coordination. It is for people whose situations are more complex and who need specialist support. A specialist Support Coordinator will assist you to manage challenges in your support environment and ensuring consistent delivery of service.
