Navigating the NDIS is tricky, and many people don’t know where to start with their plan. That’s where we come in - the best way to describe our role as Support Coordinators, is kind of like being your NDIS personal assistant or, your support coordination super hero's-capes optional, impact guaranteed.

Here is what we do:

  • Work closely with participants, and their informal, formal and mainstream supports to provide independent support, and coordinate all aspects of their NDIS plan.
  • Pride ourselves on being able to effectively research and establish links with a range of community, mainstream and funded support services as needed.
  • Provide quality, capacity building support, assisting participants to:
  • Source and access support options and connect with services
  • Coordinate services
  • Implement, monitor and manage plans
  • Build skills and confidence
  • Respond to and deal with crisis'
  • Work towards their goals
  • Report outcomes to the NDIS and assist with pre-planning prior to plan review
  • Pride ourselves on being knowledgeable, helpful and happy!

CONNECTION & COORDINATION

SUPPORT COORDINATION

 

We combine Support Coordination with Mentoring, allowing us to provide more than just the traditional duties of a Support Coordinator. This means you have access to flexible support to help achieve your NDIS goals, and build capacit. Your Support Coordinator and Mentor will work with you to use your allocated funding in the way that best supports your needs – always guided by your choice and control

There are three levels of support coordination that we can assist you with:

  • Support connection – This support is to build your ability to connect with informal, community and funded supports enabling you to get the most out of your plan and achieve your goals.
  • Support coordination (Level 2)– This support will assist you to build the skills you need to understand, implement and use your plan. We will work with you to ensure a mix of supports are used to increase your capacity to maintain relationships, manage service delivery tasks, live more independently and be included in your community.
  • Specialist Support Coordination - This is a higher level of support coordination. It is for people whose situations are more complex and who need specialist support. A specialist Support Coordinator will assist you to manage challenges in your support environment and ensuring consistent delivery of service. (See our Specialist Coordination page for more info!)
